DVIR Book · 2-Ply Carbonless · 31 Sets per Pad

FMCSA §396.11 vehicle inspection report book. 31 sets, 2-ply carbonless, every pre-trip and post-trip field in the order a driver actually walks the truck.

A DVIR pad that holds up at the roadside.

A DVIR is the form an inspector wants to see when they want proof you walked your truck. We rebuilt this pad around how working drivers actually fill it in.

Laid out the way a driver walks the truck.

Tractor inspection points on the left. Trailer inspection on the bottom. Safety equipment on the right. Defects across the top. The form follows the natural order of a pre-trip — not the alphabetical order a clerk would design.

A REMARKS box big enough to use.

Most generic DVIR pads give you a single line for defect notes. Ours gives you a real block — room to actually describe a brake light out, a slow leak, a worn fifth wheel. The mechanic on the receiving end can read what the problem actually is, not a four-word stub.

The 2-ply system covers both retention rules.

Original (white) goes to the home terminal — §396.11(c) requires retention of at least 3 months. Duplicate (yellow) stays with the driver. One form, both retention rules covered, no photocopier required.

Subscribe & Save 10% — match it to your fleet's cadence.

One pad covers about 31 days of pre-trip / post-trip inspections per truck. Pick 30, 60, or 90-day cadence, save 10%, skip or cancel from a phone.

Drivers and carriers remain responsible for completing, storing, and using records according to applicable rules and company procedures.

Drivers ask

Will this pass a DOT roadside inspection?
Every form captures the fields FMCSA §396.11 calls for, tractor, trailer, brakes, lights, tires, coupling devices, safety equipment, defects, and driver signature, so you can hand a DOT officer a clean, complete record. The form is designed to help drivers document inspection information commonly required under §396.11 and §396.13; drivers and carriers remain responsible for completing and maintaining records correctly.
Does it work for both pre-trip and post-trip inspections?
Yes. The layout handles the pre-trip review before you roll and the post-trip defect report at the end of the day, so you're not juggling two pads.
Do I still need paper DVIRs if I have an ELD?
Yes. DVIRs are separate from HOS logs. Even with an ELD for hours-of-service, FMCSA §396.11 still requires a written daily vehicle inspection report for any defects affecting safe operation.
How many inspections does one pad hold?
31 sets. A full month of daily inspections per pad.
Why is it carbonless / 2-ply?
Every inspection writes an original and a duplicate. The driver keeps one copy and the carrier files the other, which supports the FMCSA requirement to retain inspection records for three months.
What vehicles is this for?
Trucks, buses, trailers, and motorcoaches, any commercial motor vehicle under FMCSA daily inspection rules.
Can I fill it out without a desk or clipboard?
Yes. The built-in hardback backer gives you a firm writing surface, so you can complete the form standing at the truck.
Will the ink smear in the cab or in bad weather?
The quick-dry premium stock resists smearing, so entries stay legible even when you're writing fast or in the heat.
What size are the forms?
8.5″ × 5.5″ (216 × 140 mm). Compact enough for the door pocket or glovebox and fits a standard clipboard.
Do I have to submit a report every day, even with no defects?
Requirements differ between passenger and property-carrying vehicles, and many carriers require a daily record as company policy. Follow your carrier's policy and current FMCSA rules, this pad gives you a compliant form for every day you need one.
How fast do you ship?
Orders ship within 1 business day via USPS Priority or UPS Ground. Most U.S. addresses arrive within 3–5 business days.
